Welcome to Four Peaks Neurology

We hope the information you find here will help you become familiar with our Physicians, our practice, and the medical services we offer. We strive to provide the best possible service and treatment available to our patients. Please take the time to review all of the information that is available to you. You will find copies of our office forms available for download, which can be filled out at your convenience, prior to your first visit.

 

If you are new to our practice, please review our Office Policy and Procedure sheet. This information is designed to help you understand that your health care with our practice is very important to us and we want to make sure that you, our patient, understand that there are procedures that must be followed to provide you with quality care in a timely and efficient manner.

 

We respect your privacy and will keep your information confidential. Please read the Uses and Disclosures of Protected Health Information, Notice of Privacy Practices (HIPPA) that our office uses to describe how medical information about you may be used and disclosed and how you can obtain access to this information.

 

As with all medical practices, it may occasionally be necessary to share your medical information with other treating physicians and/or health care affiliates as may be needed. Medical Records Release Forms are available for this purpose.

 

We kindly ask that all of our new patients visit our First Time Visit To Our Practice page in advance of your first appointment. The page contains important information as to what you need to bring, including forms to download, print, fill-out and bring with you to your first appointment.

Neurologist Dr. Mariano Porto Joins Four Peaks

Four Peaks Neurology is proud to announce Dr. Mariano Porto is joining our team of neurologists. He comes to us from San Antonio, Texas where he was in private practice. Dr. Porto is a general neurologist specia

lizing in migraines and epilepsy. He has also worked extensively evaluating and treating dementia, strokes, seizures, tremors, Parkinson’s disease, multiple sclerosis, neuropathy, traumatic brain disease, and vertigo.

Dr. Porto received his medical degree from the University of Texas Health Science Center at San Antonio. He completed an internship in internal medicine at the University of Kansas Medical Center then a residency in neurology at the University of California Irvine Medical Center. He returned to San Antonio for his fellowship training in clinical neurophysiology. He has worked at university hospitals, Veterans Administration, and the Epilepsy Foundation.

Dr. Porto looks forward to biking, hiking, and traveling in Arizona. He’s also a fan of board games and spending time with family.
