Psychological Clearance for Pain Management, DBS, and Bariatric Procedures

First, a brief clinical interview (approximately 30 minutes) is conducted with your neuropsychologist to discuss the reason for the referral (pain or bariatric) and to collect your background history, as well as any other pertinent information.

Next, the patient will be administered psychological measures (approximately one hour) that are standardized and utilized for insurance agencies to provide clearance of surgery. The results of these measures will be interpreted by your neuropsychologist, and clearance will be determined.

The examination in total will take approximately 2 hours.

A report detailing a determination of clearance will be provided to the referring provider (pain management or bariatric) approximately 48 hours following the appointment.

The patient will follow-up with their referring provider for continuation of treatment but may contact Dr. Blais if there are any questions or concerns.

Outside Physician Requests

If you would like to schedule psychological clearance examination with our office, please fax patient demographic information (including insurance), most recent patient history, and physician notes, along with an authorization, if needed, to our office at (480) 614-0353. One of our office staff members will contact the patient for scheduling.
